Looks very similar to Bicron Square
Units in PN left of / seem to be .25"
so 5I5x12 = 1.25" square 3" long
right of / is PMT dia so about 1 1/8" dia
From circuit component values, resistance across coax
should be 11.4 Meg to 16.4 Meg as pot is adjusted.
PMT spec lists cathode to anode voltage of 700 V typical
and 1500 V max. 750 to 850 V is a typical operating
voltage.
